This is a simple customer contact that I used for performing regular scheduled maintenance. It's a good start on a contract & can easily be modified to suit your needs for the residential or commercial customer. It's in a word doc format & should easily be opened in MS Word or word pad. This is a simple customer work-check sheet that can be used by who ever performs maintenance at the property that day. It contains fields for customer location, contact info, special instructions, area to check off services performed, final property and equipment check off, place for person in charge to Initial & leave special notes about property on that particular visit.
